These establishments received the following scores for being out of compliance as stated:

(A/93) Amarillo Discount, 4000 River Road. Multiple food items expired; soda nozzles and ice machine need to be cleaned. Correct by 01/08. Floors and shelves in walk-in cooler need to be cleaned. Correct by 04/05.

(A/92) Beef O Brady’s, 7306 S.W. 34th Ave. Chocolate syrup sitting out at room temperature; sanitizer in buckets too strong (repeat violation). Correct by 01/09. Mildew build-up on soda dispenser at bar (repeat violation); drains in kitchen backing up when water is running; water at sink in men’s bathroom does not turn off completely. Correct by 04/06.

(A/98) Chick-Fil-A at Georgia Street, 2525 S. Georgia St. Large ice build-up in walk-in freezer (repeat violation); shelves, ceiling and walls in walk-in freezer need cleaning (repeat violation). Correct by 04/09.

(A/90) Chicken Express, 3514 E. I-40. Food debris on several pieces of washed equipment on drying rack. Correct by 01/08. Large cracks in lids of brine containers. Correct by 01/15. Employees without hair or beard restraints (repeat violation); paddle for brine in walk-in cooler wedged between wall and pipe that is soiled with mold and dust; containers holding any product not easily identifiable must be labeled with common name; heavily soiled cart holding equipment near chicken hot holding unit (repeat violation); mold growing on pipes and walls of walk-in coolers (repeat violation); cardboard under flour and sugar sacks must be removed (repeat violation); floors in walk in cooler beginning to come lose and allowing water to collect under metal sheet (repeat violation); grease along base of oil tank near fryers and on wall; several floor tiles broken or missing; wall behind fryers missing plastic and exposing dry wall; wall in men’s restroom must be repaired to be easily cleanable and non-absorbent (repeat violation). Correct by 04/05.

(A/94) Dollar General Store #3272, 1518 S.E. 10th Ave. Several containers of expired chicken salad in cooler; badly dented cans on shelves for sale; no soap and paper towels in men’s restroom; toilet in men’s restroom very dirty. COS. Water at sink in women’s restroom does not turn off. Correct by 04/06.

(A/96) Five Points Nursing & Rehabilitation, 1625 Point West Parkway. Debris accumulating on top of ware washing machine. Correct by 01/19. Mold in ice machine; scoop used for ice machine must be stored in a clean, dry location and not in water; single-use coffee straws must be individually wrapped or in a dispenser. Correct by 04/09.

(A/94) Hacienda La Grandeza, 10695 N. UH 287. Sanitizer must be unscented; sanitizer test strips needed at all bar areas. Correct by 01/20. Water samples must be taken every six months and sent to a lab; BOD testing must be conducted yearly and sent to a lab. Correct by 04/10.

(A/92) House Divided, 7609 Hillside Road. Trays of ice cream uncovered in walk-in freezer. COS. Quaternary ammonium sanitizer at improper concentration (repeat violation). Correct by 01/08. Personal drinks in food prep areas; bags of food stored in dirty water; ROP (reduced oxygen packaged) salmon thawing in package; establishment selling Swai fillets as catfish; household refrigerator must be replaced with commercial unit (repeat violation). Correct by 04/05.

(A/94) Impact Nutrition, 4119 Business Park Drive. Cold/cough medicine stored next to dishes. COS. Food handler certification needed; calcium build-up on side of ice machine. Correct by 07/10.

(A/98) Jiggle Berry, 3801 Olsen Blvd, Space 3. Thick layer of ice in reach-in freezer (repeat violation); large amount of burned food debris on grill press; rust and slime on juicer drainage holder. Correct by 04/09.

(F/66) La Chiquita Bakery, 3500 E. Amarillo Blvd. Chicharones in hot hold unit at improper temperature; buckets of sanitizer at improper concentration; employees not washing hands or changing gloves; owner touched ready-to-eat foods with bare hands. Correct by 01/12. One Certified Food Manager must be on premises at all times; food handler certification needed; several articles of food made on site missing labels and nutritional information; establishment’s manufacturers license has expired; heavy ice build-up in several freezers; numerous hand sinks used for dumping; grocery bags used to store food. Correct by 01/19. Live mouse in glue trap; several places throughout establishment contain pest harborage conditions and must be removed; all doors leading to outside must be sealed; mouse droppings in several areas of establishment; all wiping cloths must be stored in sanitizer solution when not in use; hair found on inside lid of container of tamales in walk-in; large bin of flour uncovered; water from thawing ice in cooler dripping directly onto sausage; burners on stove heavily coated in grease build-up; all scoops and utensils with handles must be stored so handles do not touch any food; several areas throughout establishment need to be cleaned; visible grease build-up on half of vent hood filters; water damage to ceiling tiles in front; missing tile and hole in wall in back must be repaired; most of the sinks were leaking; sign needed at hand sink indicating for hand washing only. Correct by 04/09. This is a failed inspection.

(B/88) La Frontera, 1401 S. Arthur St. Large pot of cooked rice prepared previous night at improper temperature; large batches of food must be divided into smaller quantities for proper cooling; mixers stored in hand sink; large containers of food stored on floor in walk-in cooler (repeat violation); disposable cup stored in chili powder (repeat violation). COS. Multiple areas in walk-in cooler need cleaning. Correct by 07/10.

(B/82) La Mariscada de Charly, 3500 E. Amarillo Blvd. Cut tomatoes and onions stored on counter at improper temperature. COS. Several boxes of frozen fish stacked on floor; no quaternary sanitizer in three-compartment sink. Correct by 01/12. Door to one upright freezer would not remain closed; lid on one chest freezer separating; no sanitizer test strips; no paper towels at hand sink in kitchen; sponges may not be used to clean in kitchen; box of fish and bag of octopus thawing improperly; several scoop handles in food (repeat violation); heavy mold build-up in ice maker and soda fountain nozzles (repeat violation); several areas of the ceiling throughout the kitchen visibly dirty with food debris; heavy mold build-up on fan of cooler; mops must hang upright to air dry properly (repeat violation). Correct by 04/09.

(A/92) Lupita’s Express, 2403 Hardin Drive. Household pesticide must not be used in establishment; uncovered containers of food in different coolers; door to restroom must be self-closing. COS. Popcorn texture ceiling in bathroom must be replaced to be smooth, easily cleaned and non-absorbent (repeat violation). Correct by 01/24. Filters to vent hood must be cleaned; light bulbs must be shielded, coated or shatter-resistant (repeat violation). Correct by 04/10.

(B/80) Lupita’s In N Out, 1007 Ross St. Container of meat in walk-in cooler at improper temperature; no sanitizer in three-compartment sink; no soap at any hand sinks or in bathroom. Correct by 01/09. Water pressure in restroom is too weak and not at proper temperature; ready-to-eat/TCS (temperature/time controlled for safety) foods must be date marked. Correct by 01/16. Large gap at side door must be sealed; several containers of uncovered food in walk-in; heavy grease build-up on vent hood filters (repeat violation); no lighting in employee restroom (repeat violation); restroom door must be self-closing (repeat violation). Correct by 04/06.

(A/95) Madina International Food Market, 823 Martin Road. Fish in ziplock bags in freezer must be properly labeled; knife in hand sink in back room with meat slicer (repeat violation); no soap and paper towels at hand sink (repeat violation). (A/99) Mitch’s TX BBQ #696 and Catering 1201 S. Taylor St. Grease accumulation on fryer. Correct by 04/10.

(A/92) My Thai Restaurant, 2029 S. Coulter St. Several containers of ready-to-eat and TCS foods in walk-in cooler and stand-up coolers not date marked; build-up inside doors of ware washer; personal drinks in food prep areas; packaged pasta in dry storage area must be stored at least six inches above floor; food residue accumulating in bottom of stand-up freezer; grease dripping onto equipment from vent hood. COS.

(C/74) Plum Creek Place, 6800 Plum Creek Drive. Deli meats and processed lettuce not date marked; ice scoop stored in moldy water; all staff must clean hands before preparing ready-to-eat foods. COS. Greens stored on counter; several wares in food production area stored dirty. Correct by 01/12. Two Certified Food Managers required during all times of operation; all persons entering food production area must have at least a food manager certification; all items must be date marked; cracked juice holding equipment must be removed from service. Correct by 01/19. Peeling Teflon pans must be removed; several bulk food items uncovered in dry storage; food thawing improperly; floors and walls need cleaning. Correct by 04/09.

Potatoes and nacho cheese containers stored on floor; thick layer of dust on venting in ice machine (repeat violation); several cutting boards deeply scored (repeat violation); food items in coolers not in original containers must be labeled with common name of food; two Hamilton Beach warmers must be replaced with commercial units. (A/95) Rise n Shine Donuts, 5901 S. Coulter St. Spray bottle of unknown substance must be labeled; owner cooking without hair restraints; personal items stored with consumer items in large stand-up fridge; two buckets of icing stored on floor. COS.

(A/96) Sonic Drive In, 3600 S. Washington St. TCS food in cold holding out of date. Correct by 01/16. Food crumbs in stand-up freezer (repeat violation); intake and exhaust air ducts must be cleaned and filters changed. Correct by 04/06.

(A/96) Starbucks Coffee #14191, 3512 E. I-40. Missing coving tiles must be replaced; hand washing sign needed in men’s restroom. COS. Ice maker needs cleaning; excessive trash outside receptacle. Correct by 04/06.

(A/99) Sybil B. Harrington Learning Center II, 2500 Dale St. Window trim paint chipped and peeling; vents in kitchen need to be dusted. Correct by 04/10.

(A/95) Toot ‘n Totum #12, 3365 Bell St. Employee tea stored in reach-in cooler next to customer food products; items in small reach-in cooler without lids or covering; mold build-up under milkshake machine and sides of Icee machines; heavy layer of ice on all sides of ice cream machine; spill on floor around Icee machines; handles sticky on large reach-in freezer in storage area; thick layer of old food debris on inside of door to oven; dust on all fans in walk-in cooler and ceiling of bathrooms; hot chocolate machines need to be cleaned internally daily; debris in soda fountains behind ice depositor; layers of dirt and debris between soda fountains; sign needed at hand sink in storage area indicating for hand washing only.
